Collaborative Safety is dedicated to working with human services agencies to embrace a systems approach to learning and improvement. Collaborative Safety is uniquely qualified to provide tools, training and ongoing support to agencies wanting to enhance their critical incident reviews and establish a culture of safety.

What Makes Collaborative Safety Different from Other Safety Supports Agencies?

An organization-level approach to understanding safety incidents and preventing them should be standard in safety supports agencies. However, too many take more of a forensic approach, finding who made what mistakes or who was careless. While assigning blame is often necessary, diagnosing and fixing the conditions that lead to incidents can be a better approach.

A Holistic Approach Is Important

Making a problem at the agency a learning opportunity is one key function of systemic critical incident reviews. At Collaborative Safety we also help clients modify their business processes to prevent future incidents. Our systemic reviews are different from standard reviews used in many organizations. We go beyond a superficial review and description of an event. These reviews typically end with assigning blame to an employee.

A better outcome would be an evaluation that results in a relevant change in the organization. We use insights from psychology, sociology, and organizational science to evaluate and improve systems so that problems can be avoided. Forensic interviewing and behavioral analysis are two additional tools we use to diagnose problems and improve safety in your human services organization.

These reviews are only one part of a Collaborative Safety Model. Our work with child welfare agencies also includes help in the areas of organizational alignment, working safety best practices into everyday operations, and evaluation. Our team has experience applying the model in child welfare, disability services, corrections, health and human services, human resources, and several other industries.
